1.       Enter the Tournament Event and Date.   (Example: Amistad, March 1st & 2nd).

3.       Enter the names of the team members actually fishing the tournament.   Do not enter your partner’s name if your partner is not actually fishing the tournament.

4.       Circle the tournaments and options you choose to enter.   Pre entries must be postmarked no later than one week prior to the tournament.  Total the amount for the events you have circled and submit that amount.

I have read and understand the rules for the Sun Country Bass Association tournament events.  I hereby agree to be bound by and comply with all tournament rules and federal and state laws.  I expressly assume all risk(s) associated with each tournament event and I herby release Sun Country Bass Association, the host, all sponsors, and tournament officials from all claims or injury and/or damage incurred in connection with any Sun Country Bass Association event.  By signing this application I agree to submit to any truth verification testing provided by a polygraph test and agree to any outcome of any such testing.  I have sufficient health, accident and liability insurance to cover any bodily injury or property damage incurred by myself or others as a result of my participation in any Sun Country Bass Association event.  I agree that my partner, if absent from signing this release, understands all statements, rules and regulations stated above.  By signing this release, I am stating that my partner and I both agree to these terms.

6.      Check the pre entry sign up listing when you arrive at the tournament registration site.  Sign the sheet to verify that you have checked in and that your entry has been accurately recorded.  Notify tournament officials if your entry has not been accurately recorded.  Both partners are required to sign the listing.
